Physician Assistants
Advanced Practice Nurses, including:
     Clinical Nurse Specialists
     Nurse Practicioners
     Nurse Midwives
     Registered Nurse Anesthetists

Only the DoM Faculty Affairs may contact the Medical Staff Administration Office to request hospital privileges for mid-level providers.  To begin this process, please submit the following to DoM FA:

• DoM Request for Hospital Privileges for Mid-Level Providers Form
• Copy of candidate's current CV with current email address
• Copy of candidate's degree certification
• Copy of IL License
• Copy of IL Controlled Substance License (if applicable)
• Copy of Federal DEA Certificate (if applicable)

After the above information has been received, the DoM FA will contact the Medical Staff Administration Office to request the release of an NMH application, which will be sent to the candidate via the email address provided on the CV.  Please note that the DoM has no further involvement in managing the application process for mid-level providers.  Any questions should be directed to the Medical Staff Administration Office.
