The medical service at Northwestern Memorial Hospital comprises 275 of the hospital's 720 total beds. Approximately 14,000 patients are admitted to the medical service annually. More than 70 percent of these admissions arrive through the emergency room, which logs more than 70,000 visits annually. Patients may also be admitted from the outpatient clinics of residents or faculty members. The goals of the inpatient medical service at Northwestern Memorial Hospital are to provide outstanding clinical care to patients and clinical training to residents and medical students in a rigorous academic setting. Over the past few years, emergency room visits and admissions to the medical service at NMH have continued to grow. Our inpatient medical service goals have led us to implement online inpatient notes and orders. As of August 2004, inpatient orders and notes are online. Universal access to patient charts and ordering inside and outside the hospital is expected to yield significant enhancements in patient safety and medical care efficiency. A highly extensive collection of online references is also available to assist in clinical decision making. In fact, Northwestern Memorial Hospital was recently named one of America's "100 Most Wired" hospitals by Hospitals & Health Networks.
Patients admitted to the medical service are triaged to either the Northwestern, Hospitalist or Attending services by a senior resident. Currently, approximately 65% of patients are admitted to the Northwestern service, 30% to the Hospitalist service and 5% to the Attending service. This system has allowed us to be fully compliant with team and admission caps. Resident physicians are assigned to this service. Each team is comprised of a single attending physician, one PGY-2 or PGY-3 resident, two PGY-1 residents and medical students. There are ten teams in total and the call schedule is on an every fifth night rotation. Residents present new cases to the attending at the bedside on morning rounds.
Attending physicians are selected to serve in this capacity based upon prior teaching evaluations and expertise in inpatient medicine. All admissions to the Northwestern Service are reviewed and accepted by a senior medical resident. The Northwestern Service affords residents the opportunity to provide clinical care for conditions which are complex, acute and intellectually stimulating. Resident physicians are not assigned to this service. Patients on this service are cared for by attending physicians, nurses and other support staff. Inpatient conditions with lower levels of acuity may be appropriate for this service. Resident physicians are not assigned to this service. Academic hospital-based physicians on the full-time faculty of Northwestern University Feinberg School of Medicine provide 24-hour coverage of patients on this service. This service is available at the discretion of the admitting physician and is also utilized when admission caps have been reached on the Northwestern Service.
The residency program and medical school are affiliated with the Jesse Brown Veteran's Affairs Hospital system. This consists of the Westside Location and the Lakeside Location on the Northwestern campus. The Northwestern Medical Service at the VA consists of 90 beds including those in the CCU and MICU. The Northwestern medical service at the VA has approximately 4,000 annual admissions to the medical service, and all medical subspecialty services are available, including cardiac catheterization and percutaneous transluminal coronary angioplasty. While inpatient services occur at the Westside facility, approximately 3 miles from the main campus, outpatient activities occur at the Lakeside facility which is adjacent to Northwestern Memorial Hospital and the Medical School. 
The PACS system is available on each floor of the hospital. This allows residents and faculty members to view actual X-ray films, CT scans, and Ultrasound images from multiple locations.
