Welcome to Northwestern's Department of Internal Medicine You will need to arrange a few things prior to starting: Fill out Visiting Resident Forms - Click on www.NMH.org/housestaffinfo. Fill out the NMH Confidentiality Agreement (PDF), NMH ID Badge Request Form (PDF), and Patient Information System Access Form. Additional forms will be filled out as needed. Sign up for Powerchart training -- Sign up with Cindya Ramirez 6-0960 Obtain Powerchart ID & Password -- You will need to fill out the Powerchart Access Request Form and the NMH Confidentiality Form and fax these to Jim Dersnah at 6-0209. Obtain EPIC password and training/test out by calling Janet Zimmerman at 5-6886. Obtain an NMH temporary badge by filling out the attached form and faxing to Jim Dersnah at 6-0209. The NMH ID will get you around the building and also is needed for security reasons. You will get details as to your work schedule, where to meet and specialty conferences from your specific rotation. During your rotation should also try to attend these excellent conferences: - Morning Report - Feinberg 13th floor conference room. Mon, Wed - Fri. 7:30 - 8:30.
